A resident of Lake County seeking to legally change his or her name must file an application with the Lake County Probate Court. The person must have been a Lake County resident for at least one year prior to filing the application. All cases are scheduled for hearing and publication of notice of the hearing is required in all cases.

Information and Procedure

You must be a resident of Lake County for a least one continuous year before you file your application. The papers you file should be typed. Illegible documents will be refused for filing.  All names must be complete.  Use middle names instead of middle initials.  

If you should have any questions as to how to present your case or fill in the forms, you should consult with an attorney. The Court and its Deputy Clerks cannot provide you with any information regarding how to properly handle this case beyond the information provided in the Adult Name Change Packet available below.

  • Application for Change of Name of Adult LCPC Form 21.5 with filing fee
  • Addendum to Application for Change of Name of Adult ( LCPC Form 21.5)
    • This form must be signed in the presence of a Notary Public or in front of a Deputy Clerk at the Probate Court

If Requesting Waiver of Publication: Addition $5.00 Filing FeeThe document listed below must be also submitted.  The Court will then set hearing on the Application to Waive Publication. Please refer to R.C. 2717.01(A) (4) to determine if you meet the requirements

Minor Name Change

Information and Procedure

You must be a resident of Lake County for a least one continuous year before you file your application. Illegible documents will be refused for filing.  All names must be complete. Use middle names instead of middle initials. Both applicant and minor are required to attend the hearing. 

If you are attempting to change the name of a minor, the parents of the minor including an alleged father, must consent to the name change or receive legal notice of the name change by certified mail.  The parent or alleged father must receive legal notice regardless of the name change in a newspaper or general circulation in the county.  It is your responsibility to make sure that the legal notice is correct and completed.  If the parents of the minor are divorced, a certified copy of the divorce degree must be presented with the application.

A legible copy of the minor’s certified birth certificate should be presented. The copy of the birth certificate will not be made a part of the record. In a minor name change, if the parent does not consent, the applicant must prove by clear and convincing evidence that the name of the minor should be changed.  This may require witnesses and evidence to present at the time of the hearing.

  • Photocopy of the child’s birth certificate
  • Application for Change of Name of Minor(Form 21.2) with filing fee
  • Consent to Change of Name (Form 21.4) If Applicable
    • Any natural parent who is not the applicant, must either sign a consent or be served by the court with a notice of the hearing
    • If a parent is deceased, a photocopy of that parent’s death certificate must be provided
    • If a parent’s whereabouts are unknown, see Affidavit below
  • Affidavit of Unknown Residence If Applicable
