Transitions Program

Transitions

Transitions Program is a school and Community Based Program, established in 2024, that was developed to assist youth with resources and opportunities to transition back into natural prosocial activities that promote general wellness, community engagement, and healthy choices. It will provide opportunities that are individualized to increase the chances of successful engagement and personal growth.

Both youth in diversion and probation are targeted for the Transitions Program. Eligible youth would meet with an Intake or Probation Officer to determine appropriate fit for an activity. If youth, family, and Court Officer agree, then as a team they would work to get the youth registered for the specific activity.

Grant funds are utilized to cover the Transitions Program. Attendance and engagement are
monitored weekly by the Court Officer and will be documented per Grant standards.

Transportation is provided by LakeTran as needed for all Transition Programs.

Transitions Community Based Program includes Martial Arts, Creative Fine Arts, Boxing,
Crossfit, and 4-H programming.

Martial Arts through Mentor ATA Martial Arts School: Respect is a core discipline. Youth will
learn the value of respecting themselves and others. Youth will also work toward establishing better judgement and goal setting.

Creative Fine Arts through the Fine Arts Association: Creative arts include music therapy, art therapy, and adapted programming in dance, drama, and visual arts. Through trained
professionals, youth can achieve specific goals such as attention to tasks, motor development, appropriate social interaction, language development, communication, positive self-worth, and emotional expression.

Boxing through GriffonRawl and Left 2 Chin: Boxing can have many health benefits. In additional to physical fitness, it can target mental health as well. Boxing can help kids deal with stress, anxiety, depression, and other negative emotions in a healthy way. It can also help boost self-esteem, confidence, and mood.

CrossFit through a local gym: Physical exercise is a clear benefit of CrossFit that promotes
physical and emotional wellness. However, working as a group with juveniles, their ability to learn how to overcome obstacles and set goals for themselves is significant. Also, the group setting of peers and adults to offer support, encouragement, and team building can be very powerful.

4-H Program: Is a non-formal educational youth development program. 4-H programs focus on active involvement and quality experiences which stimulate lifelong learning of values and skills. Targeted skills include leadership, communication, and collaboration. 4-H offers diverse opportunities through clubs, camps, and summer programs.

Transitions School Based Program includes organized team sports, intramural sports, marching band, and other school-based clubs or organization each school has to officer. These are very important to students, not only can it have an impact on increasing academic performance, but the potential for a student to feel connected to the school community is very valuable. Extra-curricular activities can also cultivate leadership, enhance teamwork, develop time management skills and greatly contribute to positive self-image.

A total of 129 juveniles were referred to the Transitions Program in 2025.
